I do it as well, but always attach myself in some manner to the neck strap!

I do it all the time, and have never had a problem.

If there's a concern that a piece of your equipment might fail, maybe it's time to replace that piece of equipment...

any piece of equipment can fail at any time - manfrotto tripods fail, camera's fail - you just have to invest in the equipment that is least likely to fail in the situations you will need it to work in.

That's exactly what I did almost immediately after the QR opened on my Bogen head and let the camera fall (caught by the strap before it hit the ground!)...the Bogen disappeared on the used market and I bought Linhof Profi II head and had it ever since.
