Never so far.

I attribute it to the fact that I have a spare one in the bag ;)

My imagine composition sucks. I need a heavier lens.

I hate to vote because I think I'm tempting fate, but in all the years of shooting on dozens of different bodies, I have yet to loose one.

Not sure why, but call me JJ.

I lost them so frequently on my 1DmkII and mkIIn bodies that I would buy them in bulk (10 a a time) from one of the Chinese sellers on eBay. The 1DII series eyecup is a horrible design. Haven't lost a single one with any of the 5DmkII, 1DmkIII, or 1DmkIV yet.

Never, but I'd be annoyed if i lost it middle in a shoot. Sure you can still take pictures, but it's like forgetting to bring your remote control when going out taking landscapes.

I've lost several by now (and the ones I find have broken clips). I put it down to the fact that the eyecups just don't like being attached and removed over an over - such as when one uses a right angle viewfinder.
Each time the pattern is the same; the little plastic clips weaken and the eyecup becomes loose - its then a matter of time before one of the clips breaks and the eyecup is loose in its housing (then its only moments before it will fall off somewhere;))

In fact I don't even have an eyecup at present

Tools of the trade: Canon 400D, Canon 7D, Canon 70-200mm f2.8 IS L M2, Sigma 120-300mm f2.8 OS, Canon MPE 65mm f2.8 macro, Sigma 150mm f2.8 macro, Tamron 24-70mm f2.4, Sigma 70mm f2.8 macro, Sigma 8-16mm f4.5-5.6, Raynox DCR 250, loads of teleconverters and a flashy thingy too

The eyepiece falling off is another of those little inconvenient niggles that have been with us for years, right along with the pop-up flashes that won't pop up. Apparently Canon doesn't think those issues are important enough to consider changing their designs.

I've lost a couple of eyepieces over the years, until I started putting a layer of gaffer's tape on the back of them. That makes them fit tighter and makes them less likely to fall off.

If anyone happens to find one on Little Round Top at the Gettysburg battlefield park, it's mine. That's where I lost my last one... :D

none...but i've lost & found it twice. once at some kids party where the host found it and pocketed it, and gave it back after I asked him if he saw an eye cup. The second time was in front of Guu in Vancouver, BC, in the middle of the sidewalk where it lay there a whole day until I found it, undamaged/unstepped-on.

So now I wedged it with tape where it's hella tight.

Just one...was hiking with a 1dii, noticed that I had lost it. Then Bill Nye came along the trail with it and said "is this yours?" Very odd....

1 on my 350. I do remember my cousin's chinchilla chewed through it..dont remember if I threw it away or if I lost it.

I mainly use my 350 for rigging now anyway so I have the viewfinder cover on it.
