Please Explain Exactly What Bokeh Means

Actually, I think that is a poor example to use as good bokeh, the background is very cluttered and distracting. Good backgrounds (and therefore bokeh) depend on being uncluttered, giving a pleasing look that does not distract from the main image. It is also not fully out of focus and this also makes it stand out.

Good background has nothing to do with the quality of the bokeh of a lens. A lens with good bokeh would make a cluttered background less offensive or harsh on the eyes. A lens with bad bokeh would just accentuate the distractiveness of a cluttered background by adding it's own distraction due to bad bokeh. And even with a good background, a lens with bad bokeh would make the background distracting.

bokeh as I understand it is the quality of the OOF area of a photograph. I've heard folks say that creamy bokeh is better as opposed to bokeh where the background is still distinguishiable..

The reason why people associate "good bokeh" with expensive lenses is that the wider the aperture the better... and usually lenses with wide aperture are more expensive. Of course there are some exceptions (like the 50 1.8 )...

Another thing that affects the bokeh quality is the number of aperture blades. Again; more blades = more $$$.
