Well, you don't see the grain so much on the girl, but in your image, look at the background and you can see it plain. It's still there in the busy in focus areas, but not that noticeable. Then look at the background in the one I did, you can see what NeatImage does. It cleans up all that graininess.

On the other hand, if you start with an already sharp and clean image, you don't want to use Neatimage on it, cause it will soften it overall.

BTW, I think there is a free version of NeatImage.
