Th first post looks fine at full size, but I'd pop the levels a bit.

You should try stacking layers of the original image.
1 duplicate original image then run USM.
2. Use levels layer to boost the exposure just a touch.
3. make a duplicate layer of the USM layer then run gaussian blur filter at around 1.5.
4.make the blur layer a mask layer select the paint bucket and fill the mask with black(this will hide the blur)
5. select white from the color pallet and a average size paint brush and choose 49% opacity .
6. paint in the area you wish to apply the "smooth skin" to.
This example shows this exact routine, plus the use of the healing brush on the blemish over her right eye.

Is this something like what you were going for? I think you did a little too much smoothing of her skin and a soft focus shouldn't blur as much as you had it.

Okay, I'll play.. here's mine... I do all of my skin softening with the clone tool... then I add a gaussian blur in a separate layer and erase eyes, lips, hair, etc, back in to show original...

I thought the image was dark, so I bumped levels and brightness/contrast. Also played with her eyes a bit... flattened and sharpened for web.. here's my version... Might be too light for your tast, but I like things a little lighter rather than darker...

She's really pretty and didn't need much touching up of her skin, so you can really be conservative with her, in my opinion. ;)

Oh and I messed with the color a bit too.. I think my version is too yellow or something, but there seemed to be something missing on the skin tones... I don't think I achieved perfection, and far from it, but maybe you can play with it and figure out a happy balance.. the copy I made is too red or warm or something... sorry.. didn't get what I was hoping for.

Thought I'd give it a try too; I put the original next to mine for comparison, only very subtle smoothing, but I brought out the eyes a little more using curves, lightened it with levels and added a little USM.
