I'm using GridStar.Net for mine and they've been good for me. I just had to ask them to extend the storage (for an extra monthy cost). If you take them up on their domain name offer, they'll sign ownership of the domain to your after 28 days.

And yes, they provide mySql and php. Check out the link to my site in my sig for an idea of the speed.

I would look into GoDaddy! (external link) , unbeatable prices and support. They are what we started our site on when we first came online. Their lowest package 5 GB Space and 250 GB Transfer costs $3.99 a month. Plus you get your domain name for $1.99 when you order hosting through them. One thing they are good at is uptime, 99.9% of it. We never had our site go down. Plus, if it does you can call them any day at any time.

If you are looking for a Dedicated Server, which is what I use for my site. I would recommend Servers4u (external link). They have some pretty amazing support. The only reason I could see a need for a dedicated box would be if you were using massive amounts of space and bandwidth. You pay upwards of $75.00 for a dedicated box, so it is a expensive deal.
