It's fairly easy (once you know how).

In photoshop, go into Quick Mask mode (keyboard - Q)

Select the brush tool and chose a hardness of around 60-70% or so. This will soften the edges of the selection you make, so the boundary between what's blurred and what's not won't be so harsh.

Paint over the background _ you'll see it go red in colour. Change the brush size according to the detail you're trying to paint around.

When you're done, exit quick mode (Q again), and you'll see the usual dotted line selection appear. Don't worry if the line isn't exactly around your subject, you're using a soft brush...

Use "Selection->Inverse" to inverse the selection (so the slection will be what you painted).

Then go to Filter->Blur->Gaussian Blur. Change the level of blur to what you fancy and then press OK.

Job done.

When you get used to doing that, play with using various parts of the shot being blurred to different levels to give a more natural look.
