Order Forms

I apologize if this has been posted in the past but I am moving from event photography to team photos and need to find someone who can print order forms. I made and printed my own for the events but would like to do an order form with the envelope attached so they can just tear it off. Any ideas would be appreciated.

Best idea is to print the order form right on a business envelope, or if you have more items than just use a bigger size . All the order info is on the front along with name, team name, address, etc, etc. and then some other info (the small print) is on the back of a business envelope for me right now, works perfectly. Planning on adding more products though next year so will use a bigger envelope. That way they dont need two different pieces of paper, just one envelope to put the money in.

Yep but if you need something you search until you find it no matter the age, it still applies today.
