1Dx and 70-200 F2.8 and Lee Filter case on the left. And 16-35 F4 and 24-70 f2.8ii in right, and normally a 6D body in there too. You could probably get a 300F2.8 into one of the cases if you wanted too.

Yes, they came with the bag. There's a smaller one (35 AW) that just comes with the larger of the two cases.

Here is my Lowepro 350AW. Underneath 320EX is memory cards and extra batteries plus other junk. My other stuff is on another bag (Flipside 500AW) wich I use when I need to carry my 120-300. However this is what I mostly keep with me. :)
