He offered a refund and you said NO? Sorry, but I find that doesn't make any sense at all (I am a small brained guy.)
Maybe it all makes sense, and certainly you don't owe me an explanation, but it seems that since you have a camera that is USED, and is in the old series batch, IF you have the opportunity to get a full refund, you could NOW buy a NEW camera, one that is in the NEW, FIXED, and IMPROVED batch that is now coming off the assembly line.
Without knowing more, from my perspective, there is more errors than just the 99 one.

I am small brained as I missed that he did offer an exchange one. Even then though, I would have taken the cash and bought a new one, as his replacement from Canon might be a refurb, as allowed by Canon I believe within the sales agreement. Or, it might be one that is on a shelf somewhere else manufactured prior to the fix. I'd just take the cash and get one ASAP from a dealer with new ones in stock, assuming that is possible. Yeah, the guy is cool and that is definitely worth consideration, but IF he doesn't have an estimated time for you, and he doesn't know if you will get new or refurb, etc., assuming all of that CASH is the smart move, no matter how nice he is, you got to take care of your headaches first and I'm sure he'd agree.
