Simplest way to create a panoramic:

Camera, Tripod, Remote shutter release.
Set the camera in portrait orientation
Manual Focus, WB set to cloudy (best in my opinion), mirror locked up (minimal vibrations)
No CPLs - although i have had them on mine in the past :D
Take several photos - moving the tripod head (thus the camera) a fraction but allowing sufficient "overlap" between photos
Stitch the photos together in Photoshop or Hugin (free and easy to use software).
crop and you're all set.

You can also do it handheld - just line up the horizon when you're doing it, and it can be done too.

The first one is a two shot pano and the other is a 5 shot vertical pano of Portland Headlight up close I had my 70/200 lens on set at 70
