Online printing lab for CD covers/booklets?

Hey guys. I'm probably going to be shooting some stuff for a coworker of
mine in a month or so. It is going to be for his latest CD that he's doing.

I need to figure out a good place to print. It needs to have decent prices
for small batch orders (eg nothing above 250). I need to have the back
cover and a photo booklet (like 2-8 pages + front/back obviously) done.

Any suggestions? Thanks :) Oh and its for a regular size CD case,
not those 'slim' ones that some people are using lately.

Note: You can edit my photo just tell me how you did it ;)
