I use Hill and Usher Package Choice (external link)
Coverage is through The Hartford. The prices are truly reasonable and they bind coverage before you ever actually see your first bill.

You can pretty much go to any full service insurance agent that serves businesses, as they will all offer general liability policies. A $2M policy will be around $300-$600 per year depending on your circumstances. If you will have independent contractors, assistants, or talent, you may also need to cover them as well, and that typically means you will also need workers compensation insurance too (even though they aren't employees).

I generally need to provide a certificate of insurance anytime I get a location permit, or at the request of the location I an using. Its pretty commonplace, and required by local, state, and federal agencies (parks, city streets, government buildings, etc). The insurance company will fill out the certificated and fax it to your location contact. Generally, its either free, or at a nominal fee.
