I was freelance shooting a dance event where a live band was playing.

While I was setting up a few shots of the Band performance, a man approached me, handed me a business card and expressed interest in buying some photos of the band.

He went on to explain that he is a sculptor and he would like to use my images as reference material for one of his pieces. He plans on exhibiting and selling this piece in the future.

I spoke with the band and I have signed releases.

I will probably sell him digital copies. We will meet up. He will select the images he wants and I will burn them on the spot, hand him a disk and collect my fees.

Does anyone have a standard usage contract that allows for this kind of usage that I can review and modify for? and if not what would be the best terms to allow such usage.

Or do you think a standard contract allowing unaltered reproduction of digital images for personal use only will be O.K.?

In this case, I'd put a line at the bottom of the invoice & embed the notice in the File>Info fields of the images.
