Just curious as it seems like something that can be done when using a tripod.

If I'm using a single GND, and the lighting is good, I may get lazy and forgo the holder. If I'm stacking GND's or using one in tandem with a CPL, I'll use the holder...not sure if I can hold them still for long exposures. With fast SS's I simply hand hold the NDG in front of the lens, sometimes without even a tripod. For slow SS's I use the tripod and if very slow SS then I use the tripod plus a Cokin holder. I use Hitech 2 and 2 stop ND grads. Gear List
