A while back I was browsing through the forums and came across a small failry professional model release form, it was pocket sized (at least i remember it this way) that you just download and print. I saved the link but for the life of my I cant find it when I was wanting to look at it today.

Anyways have a clue what I am talking about?

Pocket size would be pretty small print, especially if it includes a parental release for minors.
Google "model release" = 320,000 returns.

I printed my simplified model release form on the back of some of my business cards. Then I have some other cards without it.

I can hand my card to a subject after the shot, then the subject reads the front of the card. Then I ask if they will sign the back so that I can send them a free (small) print. They sign it and hand it back. Sometimes I give them another one without the form.
