Middle tones

Do you try to meter with them every time? How do you tell what is middle? I know grass can be good, bricks, old wood, etc., but what if none of that is in your shot?

I hope I am asking and wording this right .

Oh ya, if you use a grey card, how do you set it up and can it pretty much work for every shot that is fairly close?

Of course, it can sometimes depend on what you are photographing. For example, at cricket matches metering from a mid tone will blow out the whites of the clothing so I will meter for the highlights on that occasion to get those correct.
