I'm having a devil of a time getting the info on this.

When using a 40D set to manual (f/5.6, 1/60, 400ISO) and a 430EX set to standard E-TTL, is it better to have the camera set to evaluative or something else, i.e. Spot?

Anyone know how the camera reacts to the different modes>

Spot vs. Center vs. Evaluative is only for the ambient light reading!

Flash ETTL either is Evaluation or Average (set via Custom Function), irregardless of the ambient meter mode.

Some feel that Average is better than Evaluative for ETTL, because the flash meter is less often fooled by highly reflective objects in the scene which result in underexposure too frequently.

That and the fact that we have semi-control with Average over the flash knowing what it'll come out. eTTL's algorithm is unknown and pretty much a guess work.

I'm not seeing this CF setting available in my 40D, nor my 580EXII manuals.
Am I not reading them correctly?

It's there on the 30D CF settings. Look for E-TTL and it'll give you the 2 options.

Read p. 103 of the 40D owner's manual:

Flash control, Flash Firing, ETTL II = Average
