I sent my 70-200 f/4L in a couple weeks ago and all I had to provide was a copy of the original purchase receipt & the print out that Canon requires once you file a service claim. If you file the claim online it'll ask for the serial number off the lens and automatically decide if it's within warranty or not. To top it off the original receipt was not even in my name. It was the receipt provided by the person I bought it from off of here. 40D - 28-135 IS - 50 1.8 MK II - Canon 200DG - Broken Tiffen Magnum X3T

That automatic decision is based on the purchase date you entered, not the S/N. Everything else is correct. Just a copy of the receipt, which should have the same purchase date you claimed. Although I still decided to do the claim online after calling. Probably if your gear is registered, they'll have the purchase date. Most of mine isn't. In any event, they won't believe you without a copy of the receipt, and the warranty card serves no purpose John Caputo
