I've used the Kata H-14 for a few years. It is a holster style, and VERY durable. It has storage for batts, spare lenses etc. It is smallish, and you may outgrow it quickly, but its great for hauling limited gear around.

HERE is one place that has it (external link)

I'm about to get a Kata bag. I've only heard good things about them.

Also their website is quite helpful showing you exactly what gear you can fit in what bags.

For the gear you mentioned, a lot of bags will work. I didn't see Crumpler mentioned, but a 5 or 6MDH would work nicely and give you some future expandability. The Domke Jim mentioned up top is also a real good bet. Kata offers some of the best bags on the planet with undeniable protection (the Domke and Crumpler offer somewhat less padding).
Cambags.com is probably the best place to see pictures of various configurations of all the bags mentioned, plus a TON of reviews. A bit dated as far as the site design goes (using frames and all) but a great resource.
If you're like most of us, you'll find bags become one of those things that no matter what, it's just never quite perfect, so you end up buying a few. Or more than a few. I think I've had 12 and still have 5. And there's still 2 more I can think of I'd like to try.
Good luck!

I use a Stealth Reporter which has a quick access panel in the lid for lenses and so on. I use this and a Computrekker depending on what I'm doing
