do you guys shoot these in f/22 or something and everything is in focus or do you pick a different focus point each time while trying to keep the camera/tripod in the same position?

I think that depends what kind of photo you want. If there were five guys in a room, would you want a small or large DoF? You can do either and you can move around the focus as well. I think to make a really realistic looking one you should focus on one point (person perhaps) and move around. That way some "guys" are in focus, some aren't. I'm going to try this out this weekend. Looks fun!

^^ there is a link to a tutorial on page one that Dozer posted. You pretty much keep the camera on a tripod move around snap some pics and blend them together

yeah , you just pick one point of focus and move around. On the first image I posted, I set my camera at f/8 @ 1/200. You can use the timer on your camera and go in position or in my case I used my light meter with the transmitter to my pocket wizard. You can see it in my hand pretending it's a phone.

That "voices in my head" one I posted was my first/only attempt. It was easier than I thought. I just changed the opacity to fade out the other 2 shots of me. I'm going to try a harder one though maybe with my kids.
