Isn't that what the focus ring does? I don't see how it's different.
The plane you may want most of the focus on is not necessarily the plane that is parallel to the image sensor.

Other than a tilted extension tube, why would you want to tilt your macro?? For the same reasons you would want a "normal" Tilt/shift lens. The only difference is you are shooting something small.
