Yes, and yes/maybe/no.

could it have been photoshoped out quicker?? or if he had a helper that could just walk behind them clearing as they go is a quick way, kind of like cleaning as you cook :D But maybe I'm remembering too much military training. It was more just a question since I personally always liked photo's where I'm wondering "how did that get there"

regardless I still like the pic as is.

Using two flashes shooting through umbrellas, one camera left slightly behind model and one directly behind camera about 6' high.
