Two light setup
22" BD to camera right just a bit off axis
45" reflective umbrella further right to camera

there was a gridded bare hot shoe flash to camera left as accent but it did not go off :D

Pretty model, not so sure if this shot would be considered "flattering" though. I'm pretty sure if your gridden flash went off this would have been a much more eye pleasing image.
