Two light setup
22" BD to camera right just a bit off axis
45" reflective umbrella further right to camera

there was a gridded bare hot shoe flash to camera left as accent but it did not go off

Pretty model, not so sure if this shot would be considered "flattering" though. I'm pretty sure if your gridden flash went off this would have been a much more eye pleasing image.
