It is new Sandisk model 60MB/s read/write (UDMA 5 - 400x)
No rebate form to fill out!
and it is from B&H Photo ;)

not to long time ago older version Extreme IV (45MB/s read/write) was selling for $219.95 - $60 rebate = $159.95
