Ticket Template Needed

Hi Folks
Wondering if anyone can direct me or has made a 2 x 8 sport ticket template.
I would like to give the kids on our hockey team one each for xmas.

I did that last night for 2 hours.
Searched Sport Tickets, Event Tickets, Concert Tickets, Raffle Tickets, etc, till my eyes bled.
You name it, I searched it my friend.
Only things there are software to buy and print online, then buy.
Thought maybe someone here would have one made, since my CS4 skills are not up to par as many other POTN'rs here.
