I did a bit of a quickie job on this. It could be done better but it would take much more time.

It looks like you have scanned the image in - why not just scan it at a higher resolution to make it bigger?

I used just the green channel. Essentially it's a lot of cloning, healing, and painting, all done on separate layers. Adding noise as required on the painted areas to match.
The background is mostly new. Various shades on grey applied randomly with a soft brush, blurred, blended and noise added to match the original. A sepia tone at the end.

It could also be colorized... To do it right I'd have to spend a lot more time on it, but this will give you the idea.
