You can also add a lumiquest bounce

You'll definitely need the 3" extender if you're going to use any form of mini softbox on the flash.

...those larger than 5" x 7" will need the extension, but the tiny 5x7 ones would be fine.

Ahh yes, you're right if we're talking about the Camera rotator. The flash rotator will require the extender.

I have the flash rotator. But with my Metz 54MZ perched directly on the camera (not using the bracket) the flash lens is centered 6.5" above the top surface of the 17-55mm lens...so a softbox of 12" total height could be acommodated without mechanical interference to the lens! The flash rotator bracket with OC-3 cord would put the center of the flash lens up another 2.5" higher than that, even without the 3" extension. While I have a Photoflex speed ring and bracket to permit a 16x22 box to be used with my Metz 45, I have no way of mounting it for the Metz 54MZ hotshoe configuration, so I haven't found myself in need of the Newton 3" extension for softbox size reasons, only for redeye reasons.

I have the flash rotator as well. I've never used mini softboxes directly on the flash so I'll just have to trust you on this one Wilt. :D
