Technically, it's a well done image. Personally, the concept is so overdone I almost want to vomit when I see the heart-shaped hands. Enough already!

With regard to the photo itself, I think the crop is a little too tight on the right. I would like to see more hand - doesn't look right cutting it off just belopw the knuckles.

I dont think this was a concept shot that was planned out too well. Seems more like an after thought.

If it was planned the shooter would not have framed the head of the subject in such a way that it would require the hand to be cut off in such a way. If you look closely there is some hair right there near the knuckle. This suggests to me that rather than having really hairy knuckles, the subject's head was directly to the right of the hand in the shot un-cropped.
