Team and Individual Pictures

What is everyone using for team and individual pictures pricing and marketing materials? I've had some request to start shooting team and individual pictures this fall, but I having a hard time finding the right type of marketing tools.
What type of envelope/brochures are being used?
How many different pictures packages do you offer?
I do MPIX offer a ton of different sports options for print, but when is it overkill on a brochure?

I took what others were charging in our area and used that as a guide. I use a white 6x9 envelope as my order form.

I designed my order form in MS word along with a 6x9 envelope. On the front of the envelope is a spot for the athletes name, a spot for the parent to mark cash or check, and a box for me (or my assistant) to write the file number in. This way the only envelopes I have to open on the field are any that are marked "Cash"

I offer 3 packages but also individual items. Originally the individual items were only offered if a package were purchased but the president of one team parents club asked that I let people buy just an individual print since some parents cannot afford much in this economy. The last team I shot, one parent only bought a team photo, everyone else went for at least a package if not more.

Here's a company in California that one of the league photographers use. He orders the envelopes directly through them.
