I noticed this at Cinema5D forums and it looked pretty cool.

It's basically a customizable slate application with A/V sync, timecode and it displays the shooting info if you need it. Only thing that bugs me is that the quick burst of shooting info and the white video sync come so fast they are affected by the rolling shutter. Other than that it's very usable.

Well i could also see an issue with using the macbeth color chart since you would have to change your exposure for the screen and then changing it again for your scene.
