I have seen this a couple times and have asked the poster who makes it with no response. Here's a link to a picture over on FM:

What type of foot mounting is this? I like it because it gives the front of the lens support and the rubber rings reduce vibration.

Looks like it might might be a composite of several components - start with a macro focusing rail under the lens foot. The gimbal looks like possibly a Custom Brackets model. That might be a start.

it looks like a wimberly gimbal, RRS long lens support package (external link), and a RRS multipurpose rail connected to the lens foot
