Here's one I really like but my cloning skills suck to bad that I don't even know how to remove the distracting background except for the flash stand on the umbrella!

In ACR, CS5 (the only version I am familiar with), it would be easy to select the light and take a stop or three off of it. But, I rather like it. I'm more distracted by the shadow on the wall on the right side of the photo. But, in any case, it would be pretty easy to select and play with using automask and a brush.

It's all about depth of field and a lens with good bokeh.

I get great "pop" with a Canon 70-200 f4/L wide open and appropriate distances between the lens, the subject and the background. I think the pop is much better with this lens than my 18-135mm, but that lens can achieve decent results, too. But, of the two lens I currently own, I favor the f4 and use it when I can. T1i 1.6 crop body.

The borders were created via the vignetting sliders in LR.

And like Aaron said, it's all about the lighting...
And also I normally soften portrait pics by about -30 to -40 on the clarity slider so the skin looks more pleasing. Then take em to CS5 and do a local contrast adjustment and save as JPEG.
No sharpening whatsoever on most of the images of my son!
