I dont remember the set up, but there was quite a few adjustment layers to get the colour and contrast where I wanted it before the final look began to take shape. I think after everything was said and done, there were about 20 layers. More than half were adjustment layers. I used:

Curves adjustment (3 for the picture, and 2 were for the final look [colour])
Exposure adjustment (2 for final look [faded])
Colour Balance (3 for the picture)
Saturation/vibrance (1 for the picture, 2 for the final look [colour])
Levels (1 for the picture)

I remember having 16 adjustment layers in total but dont remember all the steps at this time, it was to give it all a darker cold tone except for her skin which became warmer. For the specific look of the set you are probably talking about, I have uploaded a screenshot from a psd file which had it intact. Gives you a general idea of how to do it.

But the preceding steps set up the scene for this to really make an effect. Sorry I couldn't be of better help :(
