Issue with FlexTT5 and 580exii

I had a problem doing a studio shoot last night that I've never seen before..

Canon 7D w/ MiniTT1 on hot Shoe w/ 580exii on top of that.

Another 580exii on a Flextt5 on a light stand.

I had the latest firmware on everything.

Everything was on the same setting.. Channel 1, Group A.

I had my FEC set to +1. Half way through the shooting, the 580exii on the stand start firing at 1/1 power on every shot. Turned everything off and turned it back on, and checked that everything was screwed in tight. Still happening. The flashes all said it was on ETTL on the LCD.

Dismantled the whole setup, unscrewing everything.. Screwed everything back in again and I was fine.

What could have caused that? Issue with Pocketwizards? Something lose? Is my flash going?
