Specifically the Domke F-505 Lens Case, Medium, but presumably it's the same for the whole range. The only photos I can find of these things are from the front. The attachment says shoulder strap and belt loop...is it a standard sewn both sides belt loop, or does it have a snap/hook & look bit on one end. I'm thinking about getting this for a video camera for my shoulder strap, but if it's a regular belt loop than I'll go with one of my other choices.

It's pretty much like Canon's cases for their L lenses. If you want a pouch you can attach to your bag strap, take a look at the Domke 901 pouch and use either a wrap or just some quilted fabric if you want padding on it.

The Domke lens cases I have (I don't know which model) have a large sewn loop, a ring at the top of the loop and a pair of velcro straps on the back. It would be possible to use the velcro straps to attach the case to a shoulder strap.

Those sound like the Domke F901, F902 pouches (external link). Domke also makes some specific lens cases that are very like Canon's telephoto L cases (external link), except in black.
